Patient Forms
Upon entering our office, we will welcome you as a member of our family. We will request that you complete our new patient forms. These can be completed in the office or you can download them from New Patient Center and fill them out before you arrive. This paperwork provides us with your health history and information on your condition. Consultation Next, we will update your personal history so the doctor can better serve you. Then you will have a consultation with Dr. Andy Johnson to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Johnson to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals. Examination After your consultation, you will undergo a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent. Report of Findings After reviewing your health history, goals, and examining your spine, Dr. Johnson will discuss recommendations and notify you if your condition requires care with other providers. The doctor will provide the best treatment and wellness program for your needs. |
Pain and Symptom Relief
During this phase, an average of 1-4 visits is expected. Depending on if your condition is acute or chronic will usually determine the time it takes for your symptoms to diminish. These visits are usually more frequent and are really focused on offering quick relief. Rehabilitative and Corrective Care After your symptoms have been reduced, we then will move on to the rehabilitative/corrective care phase. The frequency of these visits is reduced and we will focus on therapeutic exercise to stabilize the surrounding musculature to reduce the chance of repeat onset of the same symptoms. Stopping care too early may put the patient at risk for a relapse of symptoms. Remember, pain is often the last symptom to appear and the first to disappear. Focusing of correcting the underlying cause that brought on the pain is key. Wellness Care Whether you are currently feeling great or have reached maximum improvement following an injury, periodic regular treatment is strongly recommended. These visits are preventative and will help detect and correct any new problems before they become serious. Health conscious adults choose this type of care for themselves and their families because of the multitude of benefits. These visits are usually once a month or every other month, depending on each patient. |
